Our Structured Family Caregiving program provides support, education, and oversight on behalf of waiver members(elderly and disabled medicaid clients). The family caregiver lives in the home with the member. The family caregiver is paid for providing services for their loved one. The client and family caregiver must be biologically related or by marriage, but not include legally responsible adults such as parents of minor children or spouses of the waiver participant.
Goals of the program include supporting family caregivers through health education, telephonic counseling, and active coordination with care management. Offering option of care by a trusted family member and supporting the family caregiver.

This will decrease and eliminate falls, hospital visits, and noncompliance of daily medication. Our long term goals are to education our clients and family members. Our clients and family will be educated by licensed and certified staff members daily. Our education will provide clients with the most current evidence based on efficient ways to remain safe in a homecare setting. We will provide the knowledge needed to understand medications, disease process, and safety precautions.
